It’s important to note that simply having perfectionist tendencies or occasional intrusive thoughts does not necessarily indicate OCD. A mental health professional will use standardized criteria, such as those outlined in the DSM-5 TR, to make an accurate diagnosis.

How Does ERP treatment for OCD Work?

ERP involves two main components:

  1. Exposure – Gradually facing situations, thoughts, or triggers that cause anxiety or distress in a measured and structured way.

  2. Response Prevention – Learning to resist the urge to engage in compulsions or avoidance behaviors that typically provide temporary relief but reinforce the OCD cycle.

For example, if someone struggles with contamination OCD and feels the need to wash their hands excessively, ERP might involve gradually touching an object they perceive as "dirty" and eventually refraining from washing their hands. Over time, their brain learns that the feared consequence does not occur, and / or that they can handle the anxiety.

ERP helps people with OCD by teaching their brain new lessons about their fear. Instead of just trying to make anxiety go away, ERP helps the brain learn that scary thoughts and feelings don’t have to be dangerous. When someone faces their fears without doing their usual compulsions, their brain starts to realize that nothing bad actually happens, or if it did that they can actually handle it.

Over time, the brain gets better at handling worry and uncertainty. Instead of reacting with fear, it learns that those scary thoughts are just thoughts, and they don’t need to be acted on. ERP isn’t about making fear disappear—it’s about teaching the brain that it doesn’t have to listen to fear all the time. This way, people with OCD can feel calmer, braver, and freer in their everyday lives.
